Grinding at night, tension in the jaw, discomfort while chewing – temporomandibular joint disorders show up in many ways. That orthodontic correction with clear aligners can help surprises many people. The following case study from our practice shows how it works.
The Initial Situation
A 39-year-old patient came to us with complaints in the temporomandibular joint area. Her teeth showed clear signs of nighttime grinding, and her bite was unbalanced. Rather than looking at tooth alignment in isolation, we included the final bite and the temporomandibular joints in the treatment plan from the beginning.
The Treatment
Over approximately 16 weeks, the teeth were gradually moved into a new position using clear aligners fabricated in our practice. The goal was a state in which the dental arches mesh precisely and the temporomandibular joints are relieved. The patient’s individual chewing and closing movement was digitally recorded and incorporated into the planning – allowing us to verify the intended final bite on screen.
The Result
After completion of treatment, the patient was free of symptoms. The dental arches fit cleanly together, and the temporomandibular joints functioned in a relaxed, natural position. What began as treatment for temporomandibular joint disorders also resulted in a harmonious, well-functioning bite.

What does TMD (temporomandibular disorder) mean?
TMD is an umbrella term for functional disorders of the temporomandibular joint and chewing muscles. Typical symptoms include temporomandibular joint pain, clicking, limited mouth opening, headaches, neck or ear pain, and nighttime teeth grinding.
Can clear aligners relieve temporomandibular joint disorders?
For forms of TMD that are related to an unfavorable bite position, orthodontic treatment – such as with aligners – can position the teeth so that the temporomandibular joint is relieved. Often an occlusal splint is used at the beginning. Whether this makes sense in an individual case is determined after a functional diagnosis.
How long does TMD treatment take?
That depends on the cause and severity. In the case presented, the bite position was corrected over approximately 16 weeks. Accompanying splint therapy or physical therapy may complement the treatment.
How can I tell if my symptoms are coming from the temporomandibular joint?
Signs include pain or popping in the jaw joint, restricted or asymmetric jaw opening, frequent headaches or neck pain, and teeth grinding. A definitive diagnosis is made through an orthodontic functional analysis.
Does health insurance cover TMD treatment?
