Treatment Example: Complex Correction in Adults – Aligners with Mini-Anchorage

This treatment example shows how multiple misalignments can be corrected simultaneously with aligners – thanks to small anchorage implants in the palate.

“Your case is too complex for aligners” — some adults hear this when multiple misalignments occur together. But with small anchorage implants — so-called skeletal anchorage — clear aligners reach much further than many think. This treatment example shows how we treated a challenging initial situation in an adult patient with aligners at our practice in Cologne — entirely without fixed braces. The Initial Situation

The patient presented several misalignments at once: an open bite – the front teeth did not touch when biting down –, pronounced crowding in the lower jaw, a unilateral crossbite, and an upper first molar that could no longer be preserved. A combination that classically sounds like it requires fixed braces or even a surgical procedure. The patient’s wish, however, was for a solution as discreet and removable as possible.

The Treatment Planning

The key in such cases is stable anchorage: small titanium implants in the palate (mini-implants), against which teeth can be moved in a controlled manner without unwanted neighboring teeth shifting along. Digitally planned, they made it possible to move the posterior teeth forward in a controlled way and to close the gap of the missing molar naturally – instead of filling it with an implant. For the crowding in the lower jaw, we decided after careful consideration to extract one lower incisor to create space. The actual correction was carried out with clear aligners.

The Treatment

The mini-implants were placed in a brief procedure under local anesthesia – a step that was well tolerated by the patient. Subsequently, anchorage and aligners worked hand in hand: The aligners moved the teeth step by step, the implants provided the necessary resistance. The comfort gain compared to a fixed appliance was considerable – no visible brackets, removable for eating and oral hygiene.

The Result

In the end, the gap was completely closed, the open bite corrected, the crowding resolved, and the crossbite balanced – a harmonious, functional occlusion, achieved with the patient’s own teeth and without prosthetic replacement for the gap. Even challenging initial situations can be resolved in many cases with modern, gentle methods.

The combination of aligner therapy and skeletal anchorage requires particular expertise and is far from universally available. To be honest: Not every complex case can be solved with aligners – in certain skeletal misalignments, fixed braces or combined orthognathic treatment remains the better path, and patient cooperation must be reliable. Frequently Asked Questions about Aligners in Complex Cases

Can aligners also correct severe misalignments?

In many cases, yes – especially when combined with skeletal anchorage. This significantly expands what aligners can achieve. Whether your case is among them will be shown by orthodontic diagnostics.

What are mini-implants or skeletal anchorage?

These are small titanium pins placed temporarily in the bone – usually in the palate. They provide the teeth with a solid counterhold, so that larger, targeted tooth movements can be achieved. After treatment, they are removed.

Is placing the mini-implants painful?

The procedure is performed under local anesthesia and takes only a short time. Most patients find it well tolerated; a slight feeling of pressure in the first few days is possible.

How long does such a complex treatment take?

That depends greatly on the extent of the correction and cannot be stated as a general rule. Complex cases require patience – the digital planning provides a realistic estimate at the outset.

Aligners or fixed braces in complex cases?

Both have their place. Aligners with skeletal anchorage are an inconspicuous, comfortable option for many demanding cases; in certain skeletal misalignments, fixed braces or a combined surgical approach make more sense. We decide this together after diagnostics.
