“Your case is too complex for aligners” – some adults hear this when several misalignments come together. But with small anchorage implants – called skeletal anchorage – clear aligners can reach much further than many think. This treatment case shows how we solved a challenging initial situation for an adult patient with aligners at our practice in Cologne – without fixed braces.
The Initial Situation
The patient presented with several misalignments: an open bite – the front teeth did not touch when biting together –, severe crowding in the lower jaw, a unilateral crossbite, and an upper first molar that could no longer be preserved. A combination that typically calls for fixed braces or even surgical intervention. However, the patient’s wish was for a solution as inconspicuous and removable as possible.
The Treatment Planning
The key in such cases is stable anchorage: small titanium implants in the palate (mini-implants), against which teeth can be moved in a targeted manner without unwanted neighboring teeth shifting along. Digitally planned, they made it possible to move the posterior teeth forward in a controlled manner and close the gap of the missing molar naturally – instead of filling it with an implant. For the crowding in the lower jaw, we decided after careful consideration to extract a lower incisor to create space. The actual correction was performed with clear aligners.
The Treatment
The mini-implants were placed in a brief procedure under local anesthesia – a step that was well tolerated by the patient. Subsequently, anchorage and aligners worked hand in hand: The aligners moved the teeth step by step, the implants provided the necessary resistance. The gain in comfort compared to a fixed appliance was significant – no visible brackets, removable for eating and oral hygiene.
The Result
In the end, the gap was completely closed, the open bite corrected, the crowding resolved, and the crossbite balanced – a harmonious, functional occlusion, achieved with the patient’s own teeth and without prosthetic replacement for the gap. Even challenging initial situations can be solved in many cases with modern, conservative methods.
Why We Show This Case
The combination of aligner therapy and skeletal anchorage requires specialized expertise and is by no means offered everywhere. What are mini-implants or skeletal anchorage?
These are small titanium pins that are temporarily placed in the bone – usually on the palate. They provide teeth with a stable counterforce, enabling precise execution of larger tooth movements. After treatment, they are removed.
Is the placement of mini-implants painful?
The procedure is performed under local anesthesia and takes only a short time. Most patients find it well tolerated; a slight sensation of pressure in the first few days is possible.
How long does such a complex treatment take?
That depends strongly on the extent of the correction and cannot be stated generally. Complex cases require patience – digital planning provides a realistic estimate at the beginning.
Aligners or fixed braces for complex cases?
Both have their place. Aligners with skeletal anchorage are an unobtrusive, comfortable option for many challenging cases; for certain skeletal misalignments, fixed braces or a combined surgical approach is more appropriate. We decide this together after diagnostics.
